Start from the KnowU template and tweak it, or write one from scratch. You'll get a private link to send out and a separate private link to see what comes back.
Once you create the survey, the questions are locked in — you won't be able to edit them afterward, so give it one more look first.
Save the results link somewhere safe — it's the only way back to the answers. There are no accounts, so a lost link can't be recovered.
